Office Appointments:


  • Please arrive at least 20 minutes early so you can complete the Patient Registration forms if you have not already done so.

  • What to bring:

  • Insurance Card(s) and a Photo ID: If the appointment is for a minor, you will need to provide the legal guardian’s insurance card(s) and photo ID.

  • Medical Records: please bring relevant medical records from other physicians; results and copies of X-rays; and other imaging studies and lab tests and bring these items with you to your appointment.

  • Written lists of: Medications, herbs, vitamin supplements and over-the-counter medications you are taking.

  • A Family Member or Friend: Consider asking a friend or family member to accompany you if you need any assistance in walking, using the restroom or filling out paperwork.

  • Questions: Take the time to make a list of anything you may want to ask your physician regarding your condition.

  • Comfortable Clothes: Your condition may involve X-rays. Wear loose clothing that’s easy to take off and put on

Preoperative Instructions:


  • If scheduled for surgery, you will receive a phone call from our surgery scheduling department the day before your surgery to inform you of your arrival time and any necessary instructions.

    • If you do not receive a phone call from them by 3pm the day before your scheduled surgery, please call our office and ask for a nurse at 740-586-6828. 

  • Most operations will require pre-operative testing prior to surgery. These tests will be completed at the address below.

  • Please discontinue the use of anti-inflammatories for seven days prior to your operation. This includes medications such as Aleve (Naproxen), Advil (Ibuprofen), Voltaren (Diclofenac) and Mobic (Meloxicam).

  • If taking anti-coagulants, please consult with your surgeon on when to discontinue these medications prior to your operation. This may vary based upon your underlying medical condition.

  • You may continue Tylenol products up until the day of surgery (if not contraindicated for you). 

  • You will need a responsible driver over the age of 18 for the day of surgery.

  • Please bring your cane/crutches with you if required on the day of surgery. If you do not have a cane or crutches, please contact our office prior to surgery.
